Sub Contractor Inquiry Authorization Release: In connection with my application for sub contractor work, continued sub contractor work or promotion, I understand and agree that background inquiries may be requested by Spectrum Cruises or it's designees that will seek information as to my character and work habits, including oral assessments of my job performance, experience and abilities, along with the reasons for termination of past employment. Further, I understand and agree that Spectrum Cruises may request information from various agencies including public and private sources that contain records concerning my past activities related to my work history, civil inquiries and other experiences. Medical and workers compensation information will only be requested in compliance with the federal Americans with Disabilities Act (ADA) and any related applicable state laws. I understand this information is for the purpose of making certain I am not assigned a job function that could aggravate a previous injury or illness and will be checked only after a conditional offer of a position as a sub contractor has been extended. If so, I will be notified and be given the name of the agency or the source of information. I acknowledge that a telephonic facsimile or copy of this release shall be as valid as the original. This release is valid for all federal, state, county and local agencies and authorities. Terms of Sub-Contractor: I authorize investigation of all statements contained in this application and I certify that this information is true and correct, and release all parties from all liability for any damage that may result from this investigation. I understand that at all times, the manufacture, use sale, possession, distribution or transfer of illegal drugs, controlled substances or unauthorized use of alcohol on premises is strictly prohibited and that failure to comply will result in dismissal. I understand that misrepresentation or omission of facts called for is cause for immediate dismissal without recourse. I agree to sign a release and submit to a drug screen at a clinic designated by Spectrum Cruises and understand that confirmed positive test results will eliminate the opportunity for a position as a sub contractor. If accepted for a position as a sub contractor or training, I agree to abide by all present and future guidelines of Spectrum Cruises both with respect to performance and standards of personal conduct. If ofered a position, I understand that my contract to provide general services is for an indefinite period of time and that I have not been promised continued to be contracted for general services by Spectrum Cruises for any definite, specified period of time and that my contract for general services may be terminated at any time for any reason. I understand that completion of this general contractor application form does not constitute any type of employment agreement or contract.
